I am a standard size 6, ordered a 6 and they fit perfectly. This is my third pair of Madewell loafers in 3 different styles and they all fit me perfectly. These calf hair loafers are not as forgiving out of the box as the leather ones because I did experience some heel discomfort on the first day of wear, but I will deal with the break in because the leopard print is fashionably classy and the fit overall is great. I also have slightly wide feet with bunions and I had no issues with the toe box. Love them!

While these felt true to size and width, the toe box was a little too narrow for my gnarly bunion feet. They are absolutely beautiful and classy, but I will be exchanging them for something more comfortable than stylish. Sigh.

I can usually buy an 11 M from anywhere, and the shoe fits perfectly. With these, that was not the case. The shoes are beautiful and well made, but they just don't fit me. The toe box is way too snug and the heel rubs terribly. I even purchased heel protectors and hoped the shoes would eventually break in, but every time I wear them, I get blisters on my heels. I've tried to wear them at least twice a week for several months, hoping they would work. But after a half an hour, I have to take them off and wear a different pair of shoes. Such a shame for such a lovely shoe. And quite a bit of money to waste on a shoe I can never wear.
